Albuquerque Public Schools is seeking an experienced Home Health RN to deliver high-quality care in patients' homes across Albuquerque and Rio Rancho. You will perform SOC assessments and OASIS documentation and coordinate with the care team to improve outcomes.

Ideal candidates hold NM RN license, 2+ years total nursing experience, 1+ year in home health, strong communication, organization, and reliable transportation for local travel.

Qualifications

• Active NM RN license in good standing.
• Minimum 2 years total nursing experience.
• Minimum 1 year Home Health nursing experience.
• Minimum 1 year of SOC and OASIS experience.
• Strong clinical assessment and documentation skills.
• Ability to manage patient care independently in a home setting.

Responsabilités

• Complete SOC assessments.
• Complete accurate and timely OASIS documentation.
• Perform skilled nursing assessments and visits.
• Develop and update individualized plans of care.
• Monitor changes in patient condition and communicate with physicians and the interdisciplinary care team.
• Provide patient and family education.
• Coordinate care with therapists, physicians, caregivers, and other healthcare professionals.
• Maintain accurate, timely, and compliant clinical documentation.
