Job summary

Health Care Connectors is seeking a Registered Nurse (Home Health) with OASIS experience to provide skilled nursing care in patients\' homes. You will conduct assessments, develop care plans, and document OASIS data to support quality outcomes and reimbursement.

You will work with patients, families, and an interdisciplinary team to coordinate care, monitor progress, and educate families. Strong clinical expertise, attention to detail, and independence in home care are essential.

Qualifications

• Minimal of two years of previous Home Health experience.
• Must be available on Mondays and Fridays.
• A current license as a Registered Nurse in Connecticut
• OASIS experience required.
• Have reliable transportation and be willing to commute to provide the best in-home patient care

Responsibilities

• Perform assigned duties, including administration of medication, wound care, treatments, and procedures
• Monitor clients' conditions; reporting changes to Nurse Manager
• Follow up with, execute, and properly document doctors' orders
• Perform client assessments as necessary
• Collaborate with other professionals
• Manage home health aides or CNAs
• Weekend Rotation will be required. Typically, this is one weekend per month or less.
