Senior Solutions (Senior Solutions At Home, Inc.) is seeking an experienced Registered Nurse (RN) to join our Home Health team serving Albuquerque and Rio Rancho, NM. The role emphasizes Start of Care assessments and SOC/OASIS documentation performed in patients’ homes.

Ideal candidates will have NM RN licensure, 2+ years of nursing experience with at least 1 year in home health, and the ability to work independently while coordinating with the care team.

Qualifications

• Active New Mexico RN license in good standing.
• 2+ years total nursing experience.
• 1+ year home health nursing experience.
• 1+ year Start of Care (SOC) and OASIS experience.
• Strong clinical assessment and documentation skills.
• Ability to manage patient care independently in a home setting.
• Excellent communication, organization, and time-management skills.
• Reliable transportation and ability to travel throughout the Albuquerque/Rio Rancho service area.

Responsibilities

• Complete SOC assessments.
• Complete accurate and timely OASIS documentation.
• Perform skilled nursing assessments and visits.
• Develop and update individualized plans of care.
• Monitor changes in patient condition and communicate with physicians and the interdisciplinary care team.
• Provide patient and family education.
• Coordinate care with therapists, physicians, caregivers, and other healthcare professionals.
• Maintain accurate, timely, and compliant clinical documentation.
• Help identify changes in condition and opportunities to improve patient outcomes and prevent unnecessary hospitalization.
