Always-Best-Care-Senior-Services is seeking an experienced Registered Nurse (RN) to join our Home Health team in Albuquerque and Rio Rancho, NM. This role focuses on SOC assessments, OASIS documentation, and delivering skilled nursing visits in patients' homes.

The ideal candidate holds an active NM RN license, has 2+ years of nursing including 1 year in Home Health, and can work independently with strong clinical and organizational skills.

Qualifications

• Active New Mexico RN license in good standing.
• 2+ years of total nursing experience.
• 1 year of Home Health nursing experience.
• 1 year of SOC and OASIS experience.
• Strong clinical assessment and documentation skills.
• Ability to manage patient care independently in a home setting.
• Excellent communication, organization, and time-management skills.
• Reliable transportation and ability to travel throughout Albuquerque/Rio Rancho service area.

Responsibilities

• Complete SOC assessments.
• Complete accurate OASIS documentation.
• Perform skilled nursing assessments and visits.
• Develop and update individualized plans of care.
• Monitor changes in patient condition and communicate with physicians and the interdisciplinary care team.
• Provide patient and family education.

• Maintain accurate, timely, and compliant clinical documentation.

Registered Nurse (RN) – Home Health

Albuquerque & Rio Rancho, New Mexico

We are seeking an experienced and compassionate Registered Nurse (RN) to join our growing Home Health team serving patients throughout Albuquerque and Rio Rancho.

This position is ideal for an RN who has a strong background in home health, is comfortable working independently in the field, and has hands‑on experience completing Start of Care (SOC) assessments and OASIS documentation.

Minimum Qualifications
