Job summary

Harmony Home Health Service in Albuquerque, NM is seeking a part-time registered nurse to plan, organize and direct home care services, with emphasis on community health education and family-centered care delivered in patients' homes.

You will perform OASIS assessments, develop and revise care plans, coordinate with physicians and the interdisciplinary team, supervise home health aides, and ensure safe, effective care in collaboration with patients and families.

Qualifications

• Graduate of an accredited school of nursing with state licensure.
• 1 year nursing experience recommended; home health care experience preferred.
• Current health certificate and TB test.
• Valid drivers license and CPR certification.
• Criminal background check required.

Responsibilities

• Complete initial patient and family assessment to determine home care needs including OASIS assessments.
• Regularly re-evaluate patient nursing needs and adjust the plan of care.
• Coordinate services and make referrals as needed; supervise home health aides.
• Administer medications and treatments as prescribed; provide patient and family education.
• Communicate with physicians and the care team; document progress every 60 days.
