Job summary

Horizon Home Health and Hospice is seeking a CNA to provide in-home care across our Idaho service area. You will support personal care, daily living activities, and basic clinical tasks under the supervision of a registered nurse.

The ideal candidate holds Idaho CNA certification, a valid driver’s license, and reliable transportation to travel between visits. Home health experience is preferred to ensure quality patient care.

Qualifications

• CNA currently certified in Idaho; license required to transfer to Oregon.
• Must hold a current driver’s license and auto insurance.
• Must have reliable transportation to and from visits.
• Home Health and Hospice experience preferred.

Responsibilities

• Provide personal care: baths, back rubs, oral hygiene, shampoos, linen changes.
• Assist with dressing/undressing and maintain clean, orderly living areas.
• Assist in feeding the patient if necessary.
• Take/record vitals: temperature, pulse, respiration, BP when ordered.
• Assist in ambulation/exercise as per plan of care.
• Perform ROM and simple procedures as ordered by therapy staff.
• Remind patient to self-administer medications as directed by POC.
• Ensure safety and proper use of equipment (foot stools, rails).
• Report patient condition changes to the assigned nurse and document accurately.
