Job summary

Prudential Healthcare seeks a dedicated Home Health RN to perform skilled nursing visits, patient assessments, wound care, medications management, and care coordination in patients' homes.

You will work independently but be supported by our office and clinical leadership, traveling locally within Rowland Heights and nearby areas. An active California RN license is required, with flexible work-from-home options available.

Qualifications

• Active California RN license required.
• Home Health or acute care experience preferred.
• Strong assessment and clinical judgment.
• Reliable transportation and valid driver's license.
• Ability to work independently with strong time management.

Responsibilities

• Perform in-home skilled nursing visits
• Complete comprehensive assessments and ongoing evaluations
• Administer medications, wound care, injections, and other skilled procedures
• Update and follow individualized care plans
• Communicate with physicians and interdisciplinary team members
• Provide patient and caregiver education
• Document accurately and timely
• Travel within Rowland Heights and nearby areas
