Job summary

Prudential Healthcare seeks a dedicated Home Health RN to provide in-home skilled nursing visits, assessments, wound care, and care coordination in La Puente and surrounding communities.

Independent work supported by our office and clinical leadership, with travel required. California RN license and relevant home health or acute care experience preferred.

Qualifications

• Active California RN license required.
• Home Health or acute care experience preferred.
• Strong assessment and clinical judgment skills.
• Reliable transportation and valid driver’s license.
• Ability to work independently with excellent time management.

Responsibilities

• Perform in-home skilled nursing visits.
• Complete comprehensive assessments and ongoing evaluations.
• Administer medications, wound care, injections, and other skilled procedures.
• Update and follow individualized care plans.
• Communicate with physicians and interdisciplinary team members.
• Provide patient and caregiver education.
• Document accurately and timely.
• Travel within La Puente and nearby areas.

Position Summary
