Qualifications

• A valid RN license in good standing in the state (Pennsylvania in ad; verify for TX if needed).
• Completion from an accredited nursing program with diploma or degree.
• Minimum one year clinical experience in adult/geriatric care areas.

Responsibilities

• Conduct head-to-toe assessments and identify subtle changes in patient condition.
• Provide skilled nursing in acute and post-acute care within the home setting.
• Triages and manages urgent clinical situations independently.
• Perform medication reconciliation, teaching, and safety monitoring.
• Document wound assessment, care, and progress in the chart.
