BAYADA Home Health Care in Pennsylvania is seeking a compassionate Registered Nurse (RN) to join our home health visit team in Montgomery County. You will provide skilled nursing and rehabilitative care to clients so they can remain safely at home, working with a multi-disciplinary team.

You'll perform head-to-toe assessments, acute/post-acute care duties, wound care, medications reconciliation, and real-time clinical decisions.

Formación

• Current nursing license in good standing in Pennsylvania.
• Graduation from an accredited nursing program.
• At least one year of relevant adult/geriatric clinical experience in acute care, ED, home health, hospice, ICU, med-surg or rehab.

Responsabilidades

• Perform head-to-toe assessments and identify subtle changes in patient condition.
• Triage and manage urgent clinical situations in the home setting.
• Perform medication reconciliation, patient teaching, and safety monitoring.
• Document wound assessments, care, and progress in the chart.
