BAYADA Home Health Care, Inc. is hiring a Full Time Registered Nurse (RN) to join our home health visit team in Pennsylvania.

The RN will provide skilled nursing and rehabilitative care to clients in their homes, with a focus on delivering compassionate, high-quality care to the adult/geriatric population. Responsibilities include comprehensive assessments, medication reconciliation, wound care, and dynamic scheduling with the care team.

Qualifications

• PA nursing license required in good standing.
• Graduation from an accredited nursing program.
• At least one year of hands-on care experience with adult/geriatric populations in acute or home health settings.

Responsibilities

• Perform comprehensive head-to-toe assessments and identify changes in patient condition.
• Provide skilled nursing and rehabilitative care in patients' homes.
• Triage urgent clinical situations and coordinate with the care team.
• Administer medications, perform safety monitoring, and conduct wound care.
• Document care and outcomes using point-of-care technology.
