Job summary

BAYADA Home Health Care is seeking a Registered Nurse (RN) to join our home health visit team in Montgomery County, PA. You will be an integral member of a multidisciplinary team delivering skilled nursing and rehabilitative care to clients in their homes.

The full-time position offers a Tuesday-Saturday schedule with competitive pay and benefits. RNs will perform assessments, care plans, wound care, and medication reconciliation while supporting patient autonomy and family education.

Qualifications

• Current PA nursing license in good standing.
• Graduate from an accredited nursing program.
• At least one year of adult/geriatric care experience in relevant settings.

Responsibilities

• Conduct head-to-toe assessments and identify subtle changes in patient condition.
• Provide skilled nursing and rehabilitative care in the home setting.
• Perform medication reconciliation, teaching, and safety monitoring.
• Carry out comprehensive wound assessment, care, and documentation.
• Triage and manage urgent clinical situations independently at home.
