Job summary

Louisiana HomeCare of Raceland, a part of LHC Group and a member of the UnitedHealth Group family, is hiring an RN for home health in Raceland, LA. You will provide and direct nursing care to patients in their homes, coordinating plans with physicians and the health team.

Responsibilities include supervising home health staff, maintaining compliant documentation, and participating in on-call rotations. A valid RN license in LA, driver’s license, and CPR certification are required or pursued

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
• LA: 1+ years of clinical experience as a Registered Nurse; RN licensure must have no restrictions.
• CPR certification preferred or within 90 days of hire.
• 1+ years Home Health experience preferred.
• Ability to work independently; strong communication, writing, and organizational skills.

Responsibilities

• Provides and directs nursing care in patients' homes as prescribed by the physician and in compliance with laws and policies.
• Coordinates total plan of care with physicians and other health professionals involved in care.
• Supervises home health aides and licensed nurses; provides instruction and assigns tasks.
• Performs supervisory visits and ensures quality, documentation, and regulatory compliance.
• Participates in on-call and weekend rotation as needed.
