Job summary

Elara Caring is seeking a compassionate Physical Therapist Assistant to serve home health patients across Norwalk, Stamford, and Greenwich. You will deliver therapy under the supervision of a Physical Therapist, helping patients improve strength, balance and independence in their homes.

Travel up to 50% is required. You will collaborate with nurses, therapists and physicians, receive mentorship, and have access to tuition reimbursement and continuing education opportunities.

Qualifications

• Graduate PTA program credential required.
• Current CT PTA license required.
• At least 6 months PTA experience preferred.
• Willingness to travel up to 50% to patient homes.

Responsibilities

• Provide physical therapy treatments according to the established Plan of Care.
• Work under the supervision of a licensed Physical Therapist.
• Help patients improve mobility, strength, balance, endurance and independence.
• Monitor patient progress and report changes to supervising PT.
• Review the Plan of Care before each visit.
• Provide treatment per physician orders and standards.
• Educate patients and caregivers on home exercise programs.
• Reinforce patient goals and encourage participation in therapy.
• Complete accurate clinical documentation.
• Collaborate with nurses, therapists, physicians, and care team.

Education

Graduate of an accredited Physical Therapist Assistant program

Connecticut Physical Therapist Assistant license

6 months PTA experience preferred
