Job summary

Elara Caring is seeking a dedicated Physical Therapist Assistant for home visits. You will perform direct therapy, follow the Plan of Care, and coordinate with the supervising Physical Therapist to ensure quality patient care in home settings.

The role requires travel up to 50% and a PTA license, with opportunities for growth and a supportive benefits package within a patient‑centered home care model.

Qualifications

• Graduate of an accredited PTA program approved by the American Therapy Association.
• Current, unrestricted PTA license valid for the state of work.
• Able and willing to travel 50% of the time.

Responsibilities

• Performs direct physical therapy services in accordance with standards of practice and physician orders.
• Follows Plan of Care as established by Physical Therapist and reports status changes.
• Instructs patients and families in use of prosthetic, orthotic and assistive devices; educates on home exercise programs.
