Elara Caring is hiring a compassionate Physical Therapist Assistant to serve home health patients in lower Fairfield County, CT, from Norwalk to the New York state line, including Stamford and Greenwich. You will provide therapy under the supervision of a Physical Therapist in patients' homes, helping them improve strength, mobility and independence.

• Graduate of an accredited Physical Therapist Assistant program.
• Current and unrestricted Connecticut PTA license.
• At least six months of PTA experience preferred.
• Valid driver’s license or reliable transportation to travel between homes.

Responsabilidades

• Provide PT treatments per Plan of Care.
• Work under supervision of a licensed Physical Therapist.
• Help patients improve mobility, strength, balance, endurance, and independence.
• Monitor progress and report changes to supervising PT.
• Review Plan of Care before each visit.
• Provide treatment per physician orders and clinical standards.
• Teach patients and caregivers to safely perform home exercise programs.
• Educate on safe use of walkers, canes, wheelchairs, prosthetics, and other devices.
• Reinforce goals and encourage participation in therapy.
• Complete accurate and timely clinical documentation.
• Collaborate with nurses, therapists, physicians, and care team.

Elara Caring is hiring a compassionate Physical Therapist Assistant to serve home health patients throughout lower Fairfield County. The territory extends from Norwalk to the New York state line, including Stamford and Greenwich. As a Home Health PTA, you will provide personalized therapy in patients’ homes under the supervision of a Physical Therapist. You will help patients improve their strength, mobility, balance, safety, and confidence while building meaningful one-on-one relationships. What You’ll Do

• Provide physical therapy treatments according to the established Plan of Care
• Work under the direction and supervision of a licensed Physical Therapist
• Help patients improve mobility, strength, balance, endurance, and independence
• Monitor patient progress and promptly report changes in condition to the supervising Physical Therapist
• Review the Plan of Care before each visit
• Provide treatment according to physician orders and established clinical standards
• Teach patients and caregivers how to safely complete home exercise programs
• Educate patients on the safe use of walkers, canes, wheelchairs, prosthetics, orthotics, and other assistive devices
• Reinforce patient goals and encourage participation in therapy
• Complete accurate and timely clinical documentation
• Collaborate with nurses, therapists, physicians, and other members of the care team

What You’ll Bring

• Graduate of an accredited Physical Therapist Assistant program
• Current and unrestricted Connecticut Physical Therapist Assistant license
• At least six months of experience as a Physical Therapist Assistant preferred
• Strong communication, organization, and documentation skills
• Ability to work independently in patients’ homes
• Willingness to travel throughout Norwalk, Stamford, Greenwich, and the assigned coverage area
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Must be able and willing to travel 50%
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment

The base salary for this position is $938.00 to $42.50 per hour, based on the company’s good faith estimate at the time of posting. Actual pay will be determined based on factors such as education, experience, skills, and internal equity.
