Job summary

Elara Caring is seeking a Physical Therapist Assistant in the New Haven area to deliver high-quality home-based care. You will work under the supervision of a Physical Therapist, perform direct therapy, and adhere to planned care while traveling between patient homes.

The role requires a PTA credential, an active license, and a willingness to travel up to 50% of the time. Compensation ranges from $38 to $42.50 per hour, with full-time benefits and growth opportunities.

Qualifications

• Current, unrestricted PTA license valid for the state of work.
• 6+ months experience as a Physical Therapist Assistant is preferred.
• Able and willing to travel within branch/office coverage area.
• Must possess a valid driver's license or reliable transportation to travel between patient homes and locations.
• Must be able and willing to travel 50%.

Responsibilities

• Promotes Elara Caring's philosophy, mission statement and administrative policies to ensure quality of care.
• Performs direct physical therapy services in accordance with accepted standards of practice and certified by the patient's physician to ensure quality and continuity of care.
• Follows Plan of Care as established by Physical Therapist and reports changes in status to the supervising Physical Therapist.
• Adheres to the State Practice Act Guidelines.
• Verifies the Plan of Care prior to each visit and provides care according to physician's orders, assessment data, and established standards and guidelines.
• Incorporates patient care goals into therapy care.
• Instructs patients and families in the safe use of prosthetic, orthotic and assistive devices.
• Instructs the patient and caregiver in the therapy home program.
• Physically demanding environment; full range of motion including lifting and transferring patients.
• Potentially work irregular hours including call hours if applicable.
