Hillcrest Home Care in Omaha, NE is seeking a compassionate Physical Therapist (PT) or Physical Therapist Assistant (PTA) for part-time home-based care. You will deliver skilled therapy in patients’ homes, collaborating with an interdisciplinary team to help clients regain strength, mobility, and independence.

The role emphasizes autonomy, patient-centered care, and safe, effective home visits. A Nebraska PT or PTA license and a valid driver’s license are required; travel throughout the Omaha

Qualifications

• Graduate of an accredited Physical Therapy program (PT candidates).
• Active Nebraska Physical Therapist license required.
• Graduate of an accredited Physical Therapist Assistant program (PTA candidates).
• Active Nebraska Physical Therapist Assistant license required.

Responsibilities

• Provide physical therapy services designed to improve strength, mobility, balance, function, and overall independence.
• Evaluate patients and develop individualized treatment plans, when practicing as a Physical Therapist.
• Carry out treatment plans and interventions within the scope of practice for a Physical Therapist Assistant.
• Provide therapy services in patient homes with a focus on quality, safety, and patient-centered care.
• Educate patients and caregivers on exercises, mobility techniques, home programs, and safety strategies.
• Help patients reduce fall risk and improve confidence with everyday activities.
