Job summary

Boost Home Healthcare - Omaha is seeking a licensed physical therapist to deliver compassionate, in-home therapy that restores function and independence for patients in the Omaha area. You will conduct initial evaluations, develop plans of care, supervise PTAs as allowed, and coordinate with an interdisciplinary home health team to ensure safe, effective interventions.

A DPT or equivalent, Nebraska license, and NPTE certification are required, with 1+ year in acute/hospital rehab or outpatient

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Nebraska Physical Therapy license in good standing.
• Current or eligible NPTE certification.
• At least one year of experience in an acute care, rehab, or outpatient setting.
• Home health experience (2+ years preferred).

Responsibilities

• Help patients restore function, improve mobility, reduce pain.
• Support patients in achieving the highest possible level of physical independence.
• Participate in interdisciplinary care planning with the home care team.
• Perform initial evaluations, OASIS Start of Care (SOCs), and develop individualized plans of care.
• Administer prescribed physical therapy treatments and skilled interventions.
• Conduct 30-day reassessments and update documentation as needed.
• Provide instruction to patient and family on safe mobility and home programs.
• Supervise up to two PTAs in accordance with Nebraska state guidelines.
