Ideal candidates hold Nebraska PT or PTA licensure, a valid driver's license, and a commitment to high-quality, patient-centered home

Formación

• Graduate of an accredited Physical Therapy program required.
• Active Nebraska Physical Therapist license required.
• Graduate of an accredited Physical Therapist Assistant program required.
• Active Nebraska Physical Therapist Assistant license required.
• Valid driver's license and reliable transportation required.
• Home care/therapy experience preferred.

Responsabilidades

• Provide skilled therapy in patients' homes to improve strength, mobility, and independence.
• Evaluate patients and develop individualized treatment plans.
• Carry out treatment plans within PT or PTA scope of practice.
• Educate patients and caregivers on exercises and safety strategies.
• Monitor progress and communicate changes to the care team.
• Document visits and outcomes accurately.
• Collaborate with nurses, physicians, and therapists as part of the team.
• Manage a mobile caseload with an efficient schedule.
