Job summary

Advanced Nursing Concepts is seeking a licensed physical therapist to provide in-home rehabilitation across Inverness and surrounding areas. The role requires evaluating patients, executing the physician’s plan of care, and educating families on therapy tools.

You will document progress, coordinate with physicians, and participate in discharge planning. A valid driver's license, CPR certification, and 2+ years of home health experience are preferred.

Qualifications

• Possesses a degree from a baccalaureate or Master's program in physical therapy.
• Licensed to practice as a physical therapist within the state, if applicable.
• Two years of appropriate experience as a physical therapist. Community/home health experience is preferred.
• Demonstrates good verbal and written communication and organization skills.
• Possesses and maintains current CPR certification.

Responsibilities

• Assess each patient's functional status, muscle strength, mobility, gait, ROM, and rehab potential, including the home environment.
• Carry out the physical therapy plan of care set by the physician.
• Instruct patients and families on therapeutic appliances and prosthetic devices.
• Document care accurately and on time, including clinical and progress summaries.
• Coordinate with physicians and report patient response to treatment or changes in condition.
• Identify needs for other home health services and refer as appropriate.
• Participate in discharge planning and case conferences.
• Provide consultation to families/caregivers and inservice education to organization personnel.
• Supervise physical therapy assistants per organization policy and state regulations.
• Check in on home health aide care every two weeks when therapy is the only skilled service.
• Take on other duties as assigned
