Advanced Nursing Concepts is seeking a licensed Physical Therapist to provide one-on-one home health care across Newberry and surrounding areas. You will assess patients, implement physician-prescribed plans of care, educate patients and families on devices, and document care with progress summaries.

Role requires 2+ years of PT experience (home health preferred), strong communication skills, CPR certification, and regular travel by personal vehicle within the assigned service area.

Formación

• Candidate must hold a Bachelor's or Master's degree in Physical Therapy approved by an accredited organization.
• Licensed to practice as a physical therapist in the state of Florida.
• Minimum of two years' experience as a physical therapist; home health experience preferred.
• Strong verbal and written communication and organizational skills.
• Current CPR certification is required.
• This position requires regular travel between patient homes using personal vehicle.

Responsabilidades

• Assess patient's functional status, muscle strength, mobility, gait, ROM, and rehab potential.
• Carry out the physician's physical therapy plan of care.
• Instruct patients and families on therapeutic devices.
• Document care accurately and promptly with clinical and progress summaries.
• Coordinate with physicians and report patient responses to treatment.
• Identify needs for other home health services and refer as appropriate.
• Participate in discharge planning and case conferences.
• Provide caregiver education and inservice training.
• Supervise physical therapy assistants per policy and state regulations.
• Monitor home health aide care biweekly when therapy is the only skilled service.
• Take on additional duties as assigned.
