Job summary

Advanced Nursing Concepts is seeking a licensed Physical Therapist for a dynamic home health role across Williston, Morriston, Archer, and Bronson, FL. You will assess patients, implement physician-directed plans, educate families on devices, and document progress with diligence.

The position requires two years of experience, reliable transportation, CPR certification, and active state licensure. Travel by personal vehicle is expected throughout the service area.

Qualifications

• Must have a degree in physical therapy from an accredited program.
• Licensed to practice as a physical therapist in the applicable state.
• Two years of relevant home health or community-based PT experience preferred.
• Demonstrates good verbal and written communication and organizational skills.
• Must maintain current CPR certification.

Responsibilities

• Assess patients’ functional status, muscle strength, mobility, gait, ROM, and rehab potential.
• Carry out the physician’s plan of care.
• Instruct patients and families on appliances and prosthetic devices.
• Document care accurately and on time.
