Job summary

Advanced Nursing Concepts provides one-on-one home health care to patients in their homes, supported by field mentorship and flexible scheduling. We seek a licensed Physical Therapist (PT) who can deliver patient-centered care across Kissimmee and surrounding areas.

The role requires 2+ years of PT experience, preferably in home health, with strong communication and clinical documentation skills. Travel by personal vehicle is required; drivers must have current insurance and license.

Qualifications

• Requires a degree in physical therapy from an accredited program.
• Florida PT license is required or applicable licensure.
• Two years of physical therapy experience; home health preferred.
• Strong communication and organizational skills; CPR certification.
• Must travel by personal vehicle with valid driver's license and insurance.

Responsibilities

• Assess patients’ functional status, strength, mobility, gait, ROM, and rehab potential.
• Carry out the plan of care prescribed by the physician.
• Instruct patients and families on devices and appliances.
• Document care accurately and timely, including progress notes and summaries.
• Coordinate with physicians and report on treatment response or changes in condition.
• Identify needs for other home health services and refer as appropriate.
• Participate in discharge planning and case conferences.
• Provide caregiver education and inservice training as needed.
• Supervise PT assistants per policy and state regulations.
• Check on home health aide care when therapy is the only skilled service.
• Take on other duties as assigned.
