Job summary

FeldCare Connects seeks a dedicated Physical Therapist to provide high-quality home healthcare across all ages in Auburn, CA on an independent contractor basis. You control your schedule and coverage area, delivering one-on-one treatment with flexible hours.

Responsibilities include assessing progress, documenting outcomes, and developing plans of care, while leveraging our app Cliniconnects for efficient patient referrals and streamlined documentation.

Qualifications

• Current Physical Therapist license issued by the state Board of Physical Therapy.
• License must be active, unrestricted; a conditional, provisional, or restricted license will NOT be accepted.
• Completion of an accredited Physical Therapy program.
• Reliable personal transportation.
• Bilingual a plus.

Responsibilities

• Observe, record, and report patient response to treatment and changes to the patient's condition.
• Determine and implement the plan of care.
• Deliver quality home healthcare to patients of all ages.
