Job summary

Optimal Care, Inc. seeks a Home Health Physical Therapist to deliver one-on-one rehabilitation in patients' homes, focusing on regaining mobility and independence after surgery, injury, or illness.

You will assess functional abilities, create individualized plans, guide targeted interventions to improve mobility, balance, strength, and safety, and educate patients and caregivers alongside an interdisciplinary team.

Qualifications

• Master's or doctoral degree from an accredited Physical Therapy program.
• Current state license as a Physical Therapist.
• Minimum 1 year of experience as a licensed Physical Therapist.
• Willingness to travel daily for home visits within service area.

Responsibilities

• Assess patients' functional abilities and home environment.
• Develop individualized treatment plans.
• Guide interventions to improve mobility, balance, strength, and safety.
• Educate and empower patients and caregivers during recovery.
