Optimal Care in Michigan is seeking a licensed physical therapist to deliver home health services, providing one-on-one rehab in patients' homes, performing comprehensive evaluations and creating individualized treatment plans.

You will deliver evidence-based interventions to improve mobility, educate patients and caregivers, ensure accurate EMR documentation, and collaborate with an interdisciplinary team.

Qualifications

• Licensed PT in the United States.
• Experience delivering home health or in-home rehab is a plus.
• Strong communication and patient education skills.

Responsibilities
