Enhabit Home Health & Hospice is seeking a licensed Physical Therapist to provide patient-centered rehabilitation in the home setting. The role focuses on improving mobility, self-care, and functional skills using established therapy practices and specialty programs.

The successful candidate will bring solid clinical judgment, strong communication with patients and families, and a commitment to high-quality in-home care.

Qualifications

• Be currently licensed as a Physical Therapist (PT) in the state of employment.
• A minimum of one year of clinical experience is preferred.
• Demonstrate knowledge and skill in current therapy practice.
• Possess a valid state driver’s license and automobile liability insurance.

Responsabilités

• Examine and treat patients with physical impairments using physical modalities to improve mobility and independence.
• Help patients regain self-care abilities and daily living skills through evidence-based therapy protocols.
• Utilize Specialty Programs (Balance and Fall Prevention, Spine Safety, Total Hip/Knee Replacement, Total Shoulder Replacement) to tailor treatment plans.
