Job summary

VitalCaring Group in Melbourne, FL is seeking a Home Health Physical Therapist to support patients recovering at home. You’ll evaluate functional status, create individualized care plans, and deliver hands-on therapy to improve mobility, strength, balance, and independence.

This autonomous, patient-centered role requires excellent communication, the ability to educate patients and caregivers, and collaboration with nurses and physicians.

Qualifications

• Graduate of an accredited Physical Therapy program (DPT or Master’s).
• Current Florida PT license.
• CPR certification.
• 1–2 years of home health experience preferred.
• Valid driver’s license and reliable transportation.

Responsibilities

• Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential.
• Develop and implement individualized care plans in collaboration with physicians and the care team.
• Provide hands-on therapy to improve mobility, strength, balance, and independence.
• Educate patients and caregivers on treatment plans, exercises, and safety strategies.
• Monitor patient progress and adjust treatment plans as needed.
• Support safe transitions of care and contribute to discharge planning.
• Document patient care and progress accurately in the EMR.
• Supervise PTAs in accordance with state guidelines.
