Qualifications

• Graduate of an accredited Physical Therapy program.
• Current PT license in the state of practice.
• Minimum 1-2 years of experience in home health preferred.

Responsibilities

• Evaluate and assess patients’ functional status and rehabilitation potential.
• Develop and implement individualized care plans.
• Provide hands-on therapy to improve mobility and strength.
• Monitor patient progress and adjust treatment plans as needed.
• Supervise Physical Therapist Assistants in accordance with state guidelines.
