Job summary

Advanced Nursing Concepts is seeking a licensed physical therapist to provide one-on-one home health care, assess patients, implement physician-approved plans, and supervise assistants. You will educate families on devices, document progress, and coordinate with physicians to adjust treatment as needed.

Qualifications

• Degree in physical therapy (baccalaureate or master's) from an accredited program.
• Licensed to practice as a physical therapist in the state, if applicable.
• Two years of physical therapy experience; home health preferred.
• Strong verbal and written communication and organizational skills.
• CPR certification current.
• Must be a licensed driver with insured vehicle.

Responsibilities

• Assess functional status, mobility, gait, ROM, and rehab potential, including home environment.
• Carry out physician-approved plan of care.
• Instruct patients and families on therapeutic devices.
• Document care accurately and timely; write progress summaries.
• Coordinate with physicians and report patient response.
• Identify needs for other home health services and refer accordingly.
• Participate in discharge planning and case conferences.
• Provide caregiver education and inservice training.
• Supervise physical therapy assistants per policy and state regulations.
• Check on home health aide care every two weeks when therapy is sole skilled service.
• Take on other duties as assigned.
