Job summary

Interim HealthCare of the Twin Cities seeks a Home Care Licensed Practical Nurse (LPN) to visit patients in their homes, providing care coordination, disease and medication management, wound care, catheter and ostomy support, infection prevention, and home safety guidance.

You will work full-time days with a weekend/holiday rotation. Mileage reimbursement and generous benefits are offered, including insurance and 401k, plus CEU support and license reimbursement.

Qualifications

• Current Licensed Practical Nurse (LPN) license in MN (active/in good standing).
• 2 years of LPN experience preferred.
• Valid driver’s license, reliable transportation, and proof of auto insurance.

Responsibilities

• Participate in ongoing interdisciplinary patient assessments.
• Provide services ordered by the physician as per the care plan.
• Recognize and report changes in the patient’s condition to the RN.
• Contribute to the development and evaluation of the plan of care with patient and caregivers.
• Counsel and educate patient and family on healthcare needs.
• Complete timely and accurate clinical notes documenting patient progress.
