Zusammenfassung

Interim HealthCare of the Twin Cities is seeking a dedicated Home Care Licensed Practical Nurse (LPN) to visit patients in their homes and provide skilled nursing care. You will perform care coordination, disease and medication management, wound care, catheter management, ostomy education, infection prevention, and home safety assessments.

You will join a collaborative interdisciplinary team, enjoy autonomy in the field, and participate in a weekend/holiday rotation with a full-time schedule.

Qualifikationen

• Current MN LPN license in good standing.
• CPR certification is required.
• Valid driver’s license and reliable transportation.
• Ability to travel within geographic service area.

Aufgaben

• Participate in ongoing interdisciplinary assessment of the patient.
• Provides services that are ordered by the physician as indicated in the plan of care.
• Recognizes and reports changes in the patient’s condition to the RN.
• Participates in the development and evaluation of the plan of care in partnership with the patient, caregiver(s) and family.
• Counsels and educates the patient and family in meeting healthcare needs.
• Completes timely and accurate clinical notes addressing patient progress.
