Job description

Community-based Hospice RN Case Manager, Full-time (Orange and Chatham Counties)

Job Category: Community-based Hospice Nurse

Full-Time | Monday-Friday | 8:00 a.m. to 5:00 p.m.

This position is primarily conducted in the field, providing care to patients wherever they call home, including private residences, assisted living communities, skilled nursing facilities, and other care settings throughout our service area.

Unlike a traditional hospital or clinic-based nursing position, this role requires daily travel between patient locations, independent clinical decision-making, and the ability to build meaningful relationships with patients and families in their own environments.

If you thrive working independently, enjoy being on the move throughout the day, and value one-on-one patient care, this role offers the opportunity to practice nursing in its most personal and impactful setting.

Lead Care. Build Relationships. Make Every Day Meaningful.

Exceptional hospice care starts with exceptional nurses.

As a Community-Based Hospice RN Case Manager, you'll be more than a clinician. You'll be the leader of the care team, the advocate for your patients, and the trusted guide families rely on during one of life's most important journeys.

At Transitions LifeCare, we believe every individual deserves the quality of life they want when facing a serious illness or near life's end. Our mission is to provide compassionate care, support, and expertise to individuals and families navigating life-changing illness and end-of-life care.

We're seeking an experienced and compassionate Registered Nurse Case Manager who thrives in an autonomous field-based role while leading interdisciplinary care that puts patients and families first.

If you're looking for a nursing career where your clinical expertise, leadership, and compassion truly make a difference, we'd love to connect with you.
