Transitions LifeCare is seeking an experienced Registered Nurse Case Manager for a full-time, field-based role serving Wilson and Nash counties in North Carolina. The RN will lead hospice care teams, develop individualized care plans, and travel daily to patient homes and facilities to deliver compassionate, professional nursing care.

The ideal candidate will hold an NC RN license with CPR certification, have strong clinical judgment, and excel in patient education and interdisciplinary

  • Minimum two years of recent clinical nursing experience.

  • Reliable transportation.
  • Ability to travel throughout the service area and provide care in homes and facilities.
  • Strong assessment, care planning, and clinical decision-making skills.
  • Ability to work independently while collaborating effectively with an interdisciplinary team.
  • Strong communication and patient education skills.
  • Competent computer and documentation skills.
  • Ability to lift and/or carry up to 50 pounds.

Responsibilities

  • Lead Patient-Centered Hospice Care: Develop, implement, and manage individualized hospice care plans based on each patient's goals, needs, and preferences.
  • Serve as the Clinical Leader: Lead and coordinate the interdisciplinary care team for assigned patients; delegate and supervise team members.
  • Support Patients and Families: Build trusting relationships; educate on disease progression, symptoms, medications, and hospice services.
  • Drive Quality and Excellence: Complete timely, accurate clinical documentation; participate in quality improvement initiatives; onboard and mentor new team members.

Community-based Hospice RN Case Manager, Full-time (Wilson and Nash Counties)

Job Category: Community-based Hospice Nurse

Full-Time | Monday-Friday | 8:00 a.m. to 5:00 p.m.

This position is primarily conducted in the field, providing care to patients wherever they call home, including private residences, assisted living communities, skilled nursing facilities, and other care settings throughout our service area.

Unlike a traditional hospital or clinic-based nursing position, this role requires daily travel between patient locations, independent clinical decision-making, and the ability to build meaningful relationships with patients and families in their own environments.

What You’ll Do
Lead Patient-Centered Hospice Care
  • Develop, implement, and manage individualized hospice care plans based on each patient's goals, needs, and preferences
  • Deliver skilled nursing assessments, interventions, education, and symptom management
  • Monitor patient conditions and adjust care plans utilizing professional nursing judgment
  • Ensure all care is provided in accordance with physician orders, regulatory requirements, and hospice best practices
Serve as the Clinical Leader
  • Lead and coordinate the interdisciplinary care team for assigned patients
  • Collaborate with physicians, social workers, chaplains, hospice aides, therapists, volunteers, and community partners
  • Delegate and supervise LPNs and Hospice Aides in accordance with regulations and agency standards
  • Facilitate communication among all members of the care team to ensure coordinated, high-quality care
Support Patients and Families
  • Build trusting relationships with patients and caregivers
  • Provide education regarding disease progression, symptom management, medications, and hospice services
  • Help families navigate complex healthcare decisions with compassion and confidence
  • Serve as a consistent source of clinical expertise and emotional support
Drive Quality and Excellence
  • Complete timely, accurate, and compliant clinical documentation
  • Monitor appropriate utilization of medications, supplies, and medical equipment
  • Participate in quality improvement and performance initiatives
  • Assist with onboarding and mentoring new team members

Required Qualifications
  • Current Registered Nurse (RN) license in North Carolina
  • Minimum two years of recent clinical nursing experience
