Job summary

West Coast Wound & Skin Care is seeking a skilled Physical Therapist Assistant to provide home-based physical therapy in the Jacksonville region. You will work under the supervision of a licensed Physical Therapist to implement plans of care, monitor progress, and reinforce home exercise programs for patients in their homes.

The ideal candidate enjoys autonomy, strong communication, and patient-centered care, and is comfortable traveling to patients' homes across the service area.

Qualifications

• Active Physical Therapist Assistant license in good standing.
• Graduate of an accredited PTA program.
• Strong knowledge of anatomy, physiology, kinesiology, biomechanics, rehab principles.

Responsibilities

• Provide in-home physical therapy under supervision of a licensed PT.
• Review plan of care and goals prior to treatment.
• Perform therapeutic exercises, gait and mobility training, balance training, and neuromuscular re-education.
• Educate patients and caregivers on exercises and safety strategies.
• Maintain thorough documentation and comply with regulations.
