OhioHealth in Ohio seeks a Community Health Worker to assist patients facing non-clinical barriers, collaborating with the care team to coordinate community resources and support services. The CHW conducts individual consultations and follow-ups based on social determinants of health, patient demographics, and resource gaps, helping patients navigate treatment plans.

The role may involve home or community visits depending on risk level.

Qualifications

• High school diploma or equivalent.
• Community Health Worker Certification by an approved training program or started within 1 year of hire.
• Experience working with under privileged populations in the community.

Responsabilités

• Coordinate support services or community resources for patients and provide basic health promotion education.
• Facilitates patients adherence to treatment plans and help access affordableMedication
• Convey importance of healthy lifestyle choices (nutrition, exercise, stress mgmt.) and adverse health impacts of smoking, drinking, and drug use
• Communication with health care team: Document interactions with patients and communicate regularly with Care team; Develop and maintain relationships with community resources to ensure Coordination of care for patients
• Maintain positive relationship between OhioHealth and community resources
• Document important information ancillary to medical treatment (stressors, children, domestic violence, involvement of partner, etc)
• Administrative tasks: Serve as community liaison between OhioHealth and local community agencies. Answers phones/review messages, triages calls to determine needs and appropriate course of action
• As a High Reliability Organization (HRO), responsibilities require focus on safety, quality and efficiency in performing job duties.
