Job description

The Community Health Worker will act as a liaison and advocate between UC Health patients and community-based healthcare centers or community providers. The role involves educating, mentoring, coaching, and outreach to identify patients needing primary care, medical home services, community resources, and care coordination. The goal is to promote basic healthcare access and meet social, physical, and developmental needs for a diverse patient population.

Qualifications
  • Ohio Certified Community Health Worker Certification (CCHW) required.
  • Recently certified, licensed, or graduate within the past 12 months.
  • Minimum required experience: 0–6 months equivalent.
  • Preferred experience: 7–11 months equivalent.
Responsibilities
  • Engage in population-appropriate communication.
  • Understand growth and development milestones and tasks for patients.
  • Provide clear instructions related to treatment plans.
  • Involve family/guardian in assessment, initial treatment and ongoing care.
  • Identify physical limitations and implement interventions when necessary.
  • Recognize and respond to behavioral health concerns.
  • Interpret population-related data to plan care appropriately.
  • Address unique psychological, cultural, or religious needs.
  • Recognize signs or symptoms of abuse or neglect.
  • Identify patients requiring “medical home” services or community-based health care services.
  • Collaborate with specialty care teams and monitor tracking systems to identify patients needing medical home services and health access education.
  • Provide information, education and resources to help patients overcome barriers to healthcare and follow scheduled visits.
  • Set appointments with community medical homes, health service providers and UC Health outpatient services.
  • Offer informal counseling, social support, and culturally appropriate health education.
  • Contact patients for appointment reminders.
  • Advocate for patients and families with community medical homes and health service providers.
  • Follow up with patients and providers after scheduled appointments.
  • Facilitate conflict resolution.
  • Build and nurture relationships with community medical homes and other community-based health service providers.
  • Collaborate with UC Health clinical departments and ancillary services to ensure efficient patient transition to outpatient clinics.
  • Maintain an updated database of community medical home facilities.
  • Track outreach work and referred patients.
  • Create databases to measure community health programs’ productivity, progress, and success.
  • Benchmark internal processes to improve practice and demonstrate efficiency through data-driven metrics.
Required Skills and Knowledge
  • Effective problem solving and decision-making abilities.
  • Excellent communication skills, both verbal and written, for diverse audiences.
  • Ability to communicate with clinical staff using basic medical terminology.
  • Comfortable working in fast-paced, changing environments.
  • Knowledge of local community-based healthcare networks.
  • Proficiency with Microsoft Office (Outlook, Excel, PowerPoint).
  • Understanding of HIPAA and accepted ethical standards in healthcare.
  • Ability to work independently or with minimal supervision.
  • Strong organizational skills.
  • Bilingual in Spanish (preferred).
Work Environment

The position is primarily office-based but requires travel to hospitals and other work locations. Work hours may vary to meet business needs, and regular on‑site attendance is required.
