OhioHealth is seeking a Community Health Worker to assist patients identified with non-clinical barriers and to coordinate support services with a multidisciplinary team.

The role focuses on direct patient interactions, documentation, and liaison activities with community resources to improve access to care and adherence to plans of care. Day shifts, 40 hours per week, at OhioHealth locations.

Formación

• High School or GED (Required).
• Community Health Worker Certification by an approved training program or started within 1 year of hire.

Responsabilidades

• Direct patient support (60%). Meet with patients face to face or by telecommunication to identify non-clinical barriers.
• Coordinate support services or community resources and provide basic health promotion education.
• Facilitate patients' adherence to treatment plans and help access affordable medications.
• Maintain HIPAA compliance and document interactions with patients.
• Communicate with the care team and maintain relationships with community resources to ensure coordination of care.
• Participate in multidisciplinary care team huddles and support continuity of care.
• Serve as a community liaison between OhioHealth and local agencies.
