Job summary

ServiceSource in Wilmington, DE seeks a dedicated Direct Support Professional to create, lead, and support daily programming for participants, including academic, social, life and leisure skills. You will help develop Personal Centered ISP plans and ensure compliance with Medicaid, licensure, CARF and funding sources.

You will work with families and guardians, document progress, maintain PCP records, and participate in trainings.

Qualifications

• High School diploma or GED required.
• One year experience with adults with developmental disabilities or related field.
• Bachelor’s degree in human services field and experience preferred.

Responsibilities

• Provide active engagement and support for participants in community integration, training, leisure, communication and daily living activities.
• Develop and implement Person Centered Plans (PCP) outcomes and maintain accurate PCP records, quarterly reports and daily notes.
• Maintain a safe and clean environment; report safety or medical concerns to the Program Manager and follow protocol.
• Communicate with family members, guardians and coordinators regarding participant progress and incidents.
