Active Visions in Skokie, IL seeks a Direct Support Professional to help individuals with developmental and intellectual disabilities live fuller, more independent lives. You will provide hands-on assistance with daily routines, personal care, and skill-building in a compassionate, inclusive environment.

You will mentor, advocate, and encourage meaningful participation in the community, documenting progress and ensuring person-centered care.

Qualifications

• High school diploma or equivalent required.
• Must be at least 18 years of age.
• Valid driver’s license with reliable driving record; transportation support may be required.
• Ability to lift, bend, and assist with physical tasks safely.
• Strong communication skills and the patience to work with individuals of varying needs.
• Willingness to complete required training and certifications.
• Dependable, team-oriented attitude and commitment to person-centered care.

Responsabilités

• Assist individuals with activities of daily living, including personal hygiene, grooming, meal preparation, and mobility.
• Support development of independent living skills, encouraging growth and self-confidence at each person’s pace.
• Accompany and support individuals during community outings, appointments, and recreational activities.
• Implement individualized support plans and document progress accurately and consistently.
• Administer medications according to established protocols and training.
• Foster positive relationships built on trust, respect, and patience.
• Communicate regularly with families, team members, and supervisors to ensure coordinated, person-centered care.
• Maintain a safe, clean, and supportive living environment.
