Alternative Wellness Services is seeking an in-person care coordinator in Biddeford, ME. You will meet clients where they are—at home, in the community, and in our office—helping them access housing, benefits and healthcare while building practical care plans with clinicians and nurses.

Earn base pay hourly with monthly bonuses up to $480, plus incentive PTO and opportunities for professional growth. Training, supervision, and MHRT/C certification support are provided to help you succeed.

Formación

• Bachelor's degree in psychology, social work, human services, sociology, counseling or related field.
• Associate degree in behavioral health or human services that is on the MHRT/C preapproved list through Muskie.
• Most Maine licenses (LSW, LMSW, LCSW, LCPC, LMFT, LADC, RN, etc.).
• Completed MHRT/C coursework.

Responsabilidades

• Visit clients at home, in the community and at our office.
• Help clients find housing, keep their public benefits, and connect with health care and treatment.
• Go with clients to appointments so they can speak up for what they need (mileage reimbursed).
• Build care plans together and work alongside nurses, clinicians and fellow care coordinators.

Educación

Bachelor's degree in psychology, social work, human services, sociology or counseling

Associate degree in behavioral health or human services (MHRT/C preapproved)

Most Maine licenses (LSW, LMSW, LCSW, LCPC, LMFT, LADC, RN, etc.)
