Job summary

Alternative Wellness Services Inc is hiring a Case Manager / Care Coordinator for York County (Sanford or Biddeford areas). This is an in-person, community-based role in Maine, where you’ll meet clients at home, in the community, and at our Biddeford office, helping them build stable lives.

You can earn monthly bonuses up to $480 on top of base pay and benefit from training and growth opportunities. Qualifications include a degree in psychology or related field, Maine licenses or MHRT/C

Qualifications

• Bachelor’s degree or higher in psychology, social work, human services, sociology, counseling or related field (new grads welcome)
• Associate degree in behavioral health or human services MHRT/C preapproved list through Muskie
• Most Maine licenses (LSW, LMSW, LCSW, LCPC, LMFT, LADC, RN, etc.)
• Completed MHRT/C coursework

Responsibilities

• Visit clients at home, in the community and at our Biddeford office. No two days are the same.
• Help clients find housing, keep public benefits, and connect with health care and treatment.
• Accompany clients to appointments to advocate for their needs (mileage reimbursed).
• Build care plans with nurses, clinicians and other care coordinators.

Education

What you'll do

This is an in-person, community-based role. You'll spend your days face-to-face with clients at their homes, out in the community and at our Biddeford office, not behind a screen. You'll help people living with mental health or substance use challenges build stable, healthy lives. You're part coach, part advocate and part problem-solver.

• Go with clients to appointments so they can speak up for what they need (mileage reimbursed).
• Build care plans together and work alongside nurses, clinicians and fellow care coordinators.

You qualify with one of these:

Pay is based on credentials, experience, internal equity and budget approval. The top of the range combines qualifications-based pay placement with the maximum incentive earnings available after onboarding. Incentive payments of up to $480 per month vary, must be earned monthly based on program eligibility criteria, and are not guaranteed. Additional incentive PTO is subject to program criteria. Requirements

• MHRT-C or MHRT-C Provisional Certification: Current certification or eligibility for MHRT-C/p, with paid training available for qualified candidates.
• Independent Decision-Making: Strong ability to think independently and make sound decisions.
• Transportation: Reliable transportation for community visits.
• Background Checks: Must pass background checks and not be listed on any state or federal suspension or debarment lists.
