Job summary

Alternative Wellness Services, Inc. in Maine is seeking a care coordinator to support people living with mental health or substance use challenges. You’ll visit clients at home, in the community, and at our office, and help them access housing and MaineCare benefits while advocating for their needs.

Work with nurses and clinicians to build care plans and connect clients with health care, treatment and community resources, with paid training and opportunities for advancement.

Qualifications

• Associates degree in social work/behavioral health required or preferred.
• Bachelor's degree in psychology, social work, human services, sociology, counseling or related field; new grads welcome.
• Maine professional license or eligibility (LSW/LMSW/LCSW/LCPC/LMFT/LADC/RN, etc.).
• MHRT/C coursework completed or in process.

Responsibilities

• Visiting clients at home, in the community and at our office; you won’t be stuck at a desk.
• Helping a client find an apartment or keep their MaineCare benefits.
• Accompanying clients to doctor’s appointments to advocate for their needs (mileage reimbursed).
• Building care plans and connecting clients with health care, treatment and community resources.
• Working alongside nurses, clinicians and fellow care coordinators.

Education

Associates degree in social work/behavioral health

Bachelor's degree in psychology/social work/human services/counseling

Maine license (LSW/LMSW/LCSW/LCPC/LMFT/LADC/RN)
