Job summary

Sacramento Native American Health Center, Inc. is seeking an ECM Care Manager for an in-person, community-centered role.

You will deliver direct services and coordinate care within a multidisciplinary team, assessing clients across physical and behavioral health and social needs. You will develop and oversee Individual Care Plans and Health Action Plans, connect clients to resources, and document activities in the EHR.

Qualifications

• Prior experience in care coordination or case management (required).
• High school diploma or equivalent (required).
• Familiarity with local community resources and support services.
• Strong interpersonal skills, empathy, active listening, sound judgment, and teamwork.
• Proficient in Microsoft Office, Office 365, Teams, Zoom; comfortable with EHRs.

Responsibilities

• Deliver direct services and care coordination as part of a multidisciplinary team.
• Assess client needs across physical health, mental health, SUD, oral health, trauma history, social supports, housing, employment, and community services.
• Develop and oversee ICP and HAP.
• Provide services in-office, via telehealth, and in the field—meeting clients where they are.
• Coordinate care with providers and maintain continuity from assessment through discharge.
• Apply evidence-based approaches (Motivational Interviewing, Trauma-Informed Care, Harm Reduction).
