Job summary

Ennoble Care is seeking a full-time Patient Care Coordinator in Cincinnati, OH. You will work with clinicians, staff, and families to coordinate care, maintain clear communication, and support high-quality services in home-based primary, palliative, and hospice care.

You must be organized, compassionate, and able to navigate multiple tasks while ensuring accurate documentation and excellent customer service. On-site Monday–Friday, 8:30am–5:00pm in Cincinnati.

Qualifications

• Must be comfortable speaking on the phone for large amounts of the day.
• Compassionate and empathetic toward patients with excellent customer service.
• Ability to multitask between patients and workstreams while staying organized.
• Must pass a drug screen and background check.

Responsibilities

• Create and manage individualized patient care plans and coordinate services.
• Maintain frequent contact with patients to support care coordination and program compliance.
• Document client communications accurately in the system.
• Coordinate with primary care clinicians to ensure complete care plans.
