Job summary

Evolve Therapy Services in Kenosha, WI is seeking a compassionate Speech Language Pathologist to join our in-house Skilled Nursing Facility team on a PRN basis.

The SLP will plan, organize, and direct Speech Language Pathology services, evaluate data for treatment planning, develop goals, educate patient and staff, ensure CMS-compliant documentation in NetHealth, and coordinate FEES/MBS protocols as needed. This role emphasizes high-quality resident care and collaboration with site staff.

Qualifications

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• MUST have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure

Responsibilities

• Provide quality speech therapy services and initiate referrals when appropriate.
• Evaluate and obtain data necessary for treatment planning and implementation.
• Develop a treatment plan with target list, short-term and long-term goals.
• Instruct patients, families, and staff in speech-language procedures for carryover of remediation and goals.
• Complete CMS-compliant documentation in NetHealth EMR systems.
• Oversee and implement FEES/MBS protocols with site coordination.
• Participate in screening protocols to identify resident needs with clinical excellence.

Education
