Job summary

Evolve Therapy Services, LLC in Kenosha, WI seeks a Speech Language Pathologist for an in-house Skilled Nursing Facility. This position can be Full Time, Part Time, or PRN and focuses on delivering high-quality speech therapy services within CMS guidelines, with collaboration from the facility team.

Responsibilities include evaluating residents, creating treatment plans with measurable goals, training patients and staff for carryover, and documenting in NetHealth EMR.

Qualifications

• Master's degree in Speech Language Pathology required.
• Certificate of Clinical Competency required.
• Active SLP license in state(s) of practice required.
• Ongoing continuing education to maintain licensure and certification suggested.

• Provide quality speech therapy services and initiate referrals when appropriate.
• Evaluate and obtain data for treatment planning and implementation.
• Develop treatment plans with target lists and short/long-term goals.
• Instruct patients, families, and staff to ensure carryover of therapeutic gains.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversee FEES/MBS protocols and coordinate site programs.
• Participate in screening protocols to assess resident needs.

Job Position: Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type: Full Time, Part Time, or PRN

Job Purpose: To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions could include:

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications:

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• MUST have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification
