Job summary

Evolve Therapy Services is seeking a Speech Language Pathologist for an in-house Skilled Nursing Facility in South Range, WI. This PRN position focuses on delivering high-quality speech therapy services to residents in a partner facility.

You will evaluate, plan, and implement treatments, document in NetHealth EMR, and collaborate with the care team to achieve patient goals. Ongoing licensure and CEU maintenance are required.

Qualifications

• Master's degree in Speech Language Pathology.
• Certificate of Clinical Competency.
• Active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification

Responsibilities

• Provide quality speech therapy services and initiate referrals as appropriate.
• Evaluate and obtain data for treatment planning and implementation.
• Develop treatment plans with target lists and goals.
• Instruct patient, family, and staff in speech-language procedures for carryover.
• Complete compliant CMS-related documentation in NetHealth EMR.
• Oversee FEES/MBS protocols with site coordination.
• Participate in screening protocols to meet resident needs with clinical excellence
