Evolve Therapy Services is seeking a Speech Language Pathologist for an in-house Skilled Nursing Facility in South Range, WI. This PRN role supports resident assessment, treatment planning, and direct therapy, while ensuring CMS-compliant documentation and collaboration with the care team.

Requirements include a Master’s degree in Speech Language Pathology, Certificate of Clinical Competency, and an active SLP license in the state(s) of practice, with ongoing CEUs to maintain licensure.

Qualifications

• Master’s degree in Speech-Language Pathology.
• Certificate of Clinical Competency.
• Must have active SLP license in state(s) of practice.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Provide quality speech therapy services and initiate referrals when appropriate.
• Evaluate and obtain data necessary for treatment planning.
• Develop a treatment plan with target list, short-term, and long-term goals.
• Instruct patient, family, and staff in appropriate speech-language procedures for carryover.
• Complete CMS-compliant documentation from CMS expectations in NetHealth EMR systems.
• Oversee and implement FEES/MBS protocols with site coordination.
• Participate in screening protocols to ensure needs of residents are met timely.
